Lateral Rehabilitation N. 65th Street / Eagle Street

The City of Wauwatosa will receive sealed proposals for the N. 65th St./Eagle St. lateral rehabilitation until 11:01 A.M. Local Time, Thursday, May 3, 2012, at the office of the City Clerk in the City Hall, 7725 W. North Avenue, Wauwatosa, Wisconsin 53213, at which time all bids will be publicly opened and read aloud in Committee Room No. 1 at the City Hall, Wauwatosa, Wisconsin. CONTRACT 12-99 LATERAL REHABILITATION N. 65TH ST./EAGLE ST. Under this proposal, the Contractor shall furnish all labor, materials, supplies, equipment, tools and other services necessary for: 1) Closed-circuit televising (CCTV) and locating of laterals; 2) Installation of cleanouts; 3) Trenchless CIPP lining of laterals, including both one-piece mainline and lateral liners and standard lateral liners; and 4) Soaker hose testing and dye water testing of laterals. The Contractor shall perform this work within two blocks along Eagle St., between 80th St. and Wauwatosa Ave. and within three blocks along 65th St., between North Ave. and Clark St. in accordance with contract documents, including plans and specifications, on file in the office of the Director of Public Works, Wauwatosa, Wisconsin. Work under this contract shall be performed incrementally in steps to allow testing of the effectiveness of each step of work to reduce leakage into laterals. Sequencing of the steps is shown on the Drawings. Under a separate contract, the City may also perform electro-scan testing of laterals after installation of cleanouts but prior to lining of laterals. The Contractor shall provide the City access to laterals for this testing for a minimum period of 5 working days between installation of the cleanouts/pre-construction inspection of the laterals and lining of the laterals. Work in the Eagle St. area shall be coordinated with lining of the Eagle St. main sewer, which will be completed under a separate contract. Lining of the main sewer is anticipated to be completed by June 8, 2012. Interested parties may obtain copies of the contract documents, including plans and specifications, at the office of the Director of Public Works, City Hall, 7725 W. North Avenue, Wauwatosa, Wisconsin. There is a nonrefundable charge of $25.00 for the plans and contract documents. The City will not send contract documents to local contractors or suppliers. However, the City will send out contract documents to non-local firms provided a FedEx number accompanies the request. The firm requesting the contract documents shall pay all costs for the shipping. Contractors shall address their bids to the City Clerk of the City of Wauwatosa, Wisconsin, and marked "Bid for Contract 12-99 Lateral Rehabilitation – N.65th St./Eagle St.". There will be a Pre-Bid Meeting and Site Tour on April 23, 2012 at 10: 00 A.M. that will be held at Wauwatosa City Hall. All proposals shall be submitted on a form provided for that purpose, together with a certified check or bid bond equal to five (5) percent of the bid payable to the City of Wauwatosa, Wisconsin, as a guarantee that if his bid is accepted, the successful bidder will execute and file the proper contract and bonds within ten (10) days after notification of award of the contract. In case the successful bidder shall fail to execute such contract and bonds, the amount of the check or bid bond shall be forfeited to the City as liquidated damages. Contractors and Subcontractors shall pay not less than the higher prevailing wage rate for a job classification on public work as established by the State of Wisconsin Department of Workforce Development. The Contractor may refer to s. DWD 290.10 of the Wisconsin Administrative Code and either s. 66.0903 or s 103.49, Stats. for a complete explanation of the administrative review process. Copies of these wage rates are contained in Section 400 of the Contract Documents. The letting of the work described herein is subject to the provisions of Section 66.09, Wisconsin Statutes, requiring the bidder to furnish proof of responsibility. Bidder prequalification is required on forms furnished by the City of Wauwatosa and submitted to the Director of Public Works. Prequalification forms that are submitted after 5 days preceding the contract letting date may be cause for the rejecting of bids. In addition to submitting this form, bidders must meet supplemental prequalification requirements for this project, as specified in the Contact Documents in Section 200 – INSTRUCTIONS TO BIDDERS. Bidders wishing to be considered for this Contract shall submit documentation of the specified supplemental prequalification requirements to the Director of Public Works no later than 4:00 P.M. Local Time on April 26, 2012. Prospective bidders will be notified whether they meet these prequalification requirements prior to the bid opening date. Bids received from bidders not meeting these prequalification requirements will not be opened. Contractors and/or Sub-contractors bidding will be required to use the method and specified products for conducting the work, as detailed in Section 600. TIME OF COMPLETION Work under this contract shall be completed in all respects within 170 calendar days after the City issues the Notice to Proceed. Soaker testing/dye water testing shall be completed by October 31, 2012. There shall be no other extension of time and no extenuating circumstances for any of the milestones, except for an industry strike, or the inability of the City to receive plan and specification approval. Failure on the part of the successful bidder to execute his contract and performance and labor & material payment bonds within ten (10) days from the date of notice of the award of contract will be considered as just cause for the annulment of the award and the forfeiture of the proposal guarantee to the City not as a penalty but in payment to the City as liquidated damages as a result of such failure. No bid shall be withdrawn after the opening of bids for a period of sixty (60) days after the scheduled time of closing of bids. If the Contractor does not complete the work on or before the date set forth above for Contract 12-99 Lateral Rehabilitation – N. 65th St./Eagle St. or within the extra time allowed under a Director of Public Works granted time extension, the City will assess liquidated damages. The City will deduct Eight Hundred Ninety Dollars ($890) for every calendar day that the work remains uncompleted from payments due the Contractor. The Contractor will also be charged for each and everyday inspection is required after the time of completion has expired. This charge will be based on the actual costs of inspection, construction supervision, clerical and administrative costs, traffic control and overhead charges. The Contractor shall develop, implement and maintain a substance abuse prevention program that complies with State of Wisconsin Act 181 (Chapter 103.503 of the State Statutes) and Sections 200.21 and 505.09 of this contract. The program shall cover all union and non-union employees who work on City of Wauwatosa construction sites. A required "Affidavit of Compliance" is included in Section 300 and shall be submitted with the bid. The right to reject or accept any or all bids and the right to waive any informality in bidding is reserved to the City of Wauwatosa, Wisconsin.
