SECTION 100 - OFFICIAL NOTICE OFFICIAL NOTICE TO CONTRACTORS ADVERTISEMENT FOR BIDS Sealed proposals will be received by the City of Wauwatosa until 11:01 A.M. Local Time, Thursday, August 12, 2010 at the office of the City Clerk in the City Hall, 7725 West North Avenue, Wauwatosa, Wisconsin, at which time all bids will be publicly opened and read aloud in the Committee Room, City Hall, Wauwatosa, Wisconsin. CONTRACT 10-06 ASPHALT RESURFACING To furnish all labor, materials and equipment for asphalt repaving including milling, spot concrete curb and gutter replacement, asphalt pavement, and work incidental thereto in: Street & Location Const. Type N. 120th St. from Locust St. to Chambers St. C-Asph Glenway Ct. from 110th St. to Glenway St. C-Asph Glenway St. from Glenway Ct. to Ruby Ave. C-Asph all in accordance with contract documents, including plans and specifications on file in the Engineering office of the City of Wauwatosa. Copies of the contract documents, including plans and specifications may be obtained at the Engineering office at City Hall, Wauwatosa, Wisconsin. There will be a nonrefundable Twenty-five Dollar ($25.00) charge for plans and contract documents. Requests for plans and specifications out of the local area will be mailed upon receipt of a FedEx number and charged to the recipient's account. Requests in the local area will not be mailed. Bids shall be addressed to the City Clerk of the City of Wauwatosa, Wisconsin and marked "Bids for Contract 10-06 ASPHALT RESURFACING". All proposals must be submitted on a form provided for that purpose, together with a certified check or bid bond equal to five (5) percent of the bid payable to the City of Wauwatosa, Wisconsin, as a guarantee that if his bid is accepted, the successful bidder will execute and file the proper contract and bonds within ten (10) days after notification of award of the contract. In case the successful bidder shall fail to execute such contract and bonds, the amount of the check or bid bond shall be forfeited to the City as liquidated damages. Contractor shall pay not less than the wage rates set forth in the proposed contract which have been established in accordance with the provisions of Section 66.0903, Wisconsin Statutes and Section 400 of the Contract Documents. The letting of the work described herein is subject to the provisions of Section 66.09, Wisconsin Statutes, requiring the bidder to furnish proof of responsibility. Bidder prequalification is required on forms furnished by the City of Wauwatosa and submitted to the City Engineer. Prequalification forms submitted after 5 days preceding the contract letting date shall be cause for the rejecting of bids. The date for completion shall be November 15, 2010. No bid shall be withdrawn after the opening of bids for a period of sixty (60) days after the scheduled time of closing of bids. Liquidated damages for failure to complete the work before the dates set forth above shall be Five Hundred Dollars ($500.00) for each day in excess thereof. The Contractor will be charged for each and every day inspection is required after the time of completion has expired. This charge will be based on the actual costs of inspection, construction supervision, clerical and administrative costs, traffic control and overhead charges. Please attach this addendum to the specifications in your possession. The purpose of this addendum is to modify the Contract documents as follows: The bid opening date is changed from Thursday, August 12, 2010 to Thursday, August 26, 2010. The August 12 date is listed in Section 100 and Section 300. Bid opening time remains the same.
