A solicitation will be issued seeking appropriately qualified vendors to provide clinic-based outpatient treatment services to uninsured individuals with developmental disabilities in NYC. Services would include evaluation services, habilitation services, and/or medical treatment services. Medical treatment services would include primary care, specialty medical services, dental services and/or behavioral health treatment services for people with developmental disabilities. The goals and objectives of this solicitation are as follows: • Provide outpatient clinic services at New York State certified Article 16 clinic programs in NYC for individuals with developmental disabilities who are uninsured. • Promote the use of best practices for clinic services provided to individuals with developmental disabilities. • Improve the health of individuals with developmental disabilities. • Monitor service delivery targets and desired outcomes of individuals who are served by this program. • Optimize use of available resources through monitoring of data on requests for services, program eligibility and enrollment, referrals and linkages and utilization of services. • Identify and share knowledge on effective use of evidence-informed, evidence-based, and best practices and results of quality improvement efforts among provider partners and other stakeholders. • Generate aggregate reporting measures to monitor clinical quality and public health outcomes for individuals served at Article 16 clinics. Services would be provided by appropriately licensed or certified practitioners that operate New York State certified Article 16 clinic programs. Proposers should have successful experience and commitment to providing outpatient clinic services to uninsured individuals who have developmental disabilities in NYC; experience in assisting uninsured eligible populations to obtain Medicaid or other public insurance; and the capacity to leverage existing alliances and/or existing services and systems in order to efficiently serve individuals and promote delivery of comprehensive medical care.
