BULLETIN #1 REQUEST FOR STATEMENT OF QUALIFICATIONS (RFSQ) FIREARMS AND TOOL MARK EXAMINATION AND/OR TRAINING SERVICES RFSQ – FIREARMS AND TOOL MARK SERVICES - 2004 INTRODUCTION The Los Angeles County Sheriff's Department is issuing this Request For Statement of Qualifications (RFSQ) to solicit responses from qualified vendors for Firearms and Tool Mark Examination and/or Training Services. Vendors must meet the Minimum Qualifications stated in Section 1.3 of the RFSQ and be able to demonstrate their ability to successfully meet the requirements outlined in Appendix A, Statement of Work. RFSQ TIMETABLE The timetable for this RFSQ is as follows: - Release of RFSQ................................ November 24, 2004 - Written Questions Due........................ December 7, 2004 - Request for Solicitation Requirements Review due........December 10, 2004 - Questions and Answers Released.............December 14, 2004 - SOQ due by 3:00 p.m. (Pacific Standard Time)........ December 21, 2004 RFSQ SUBMISSION INFORMATION RFSQ Release The RFSQ will be available to all interested vendors on November 24, 2004 at 2:00 p.m. in hard copy. The RFSQ will be mailed on request or may be picked up at the following address: Los Angeles County Sheriff's Department Contracts Unit, Room 214 4700 Ramona Boulevard Monterey Park, California 91754 If the RFSQ is to be mailed, Vendors must contact Irma Santana, Sheriff's Department Contracts Unit via email at [email protected] or FAX at (323) 415-6321 to indicate to whom the document should be addressed and whether the RFSQ is to be sent via priority mail. SOQ Submission Requirements The original SOQ and three (3) copies, including all requested information, required Exhibits and Attachments, shall be enclosed in a sealed envelope, clearly marked in the upper left-hand corner with the name and address of the Vendor and bear the words: "RFSQ – FIREARMS AND TOOL MARK SERVICES - 2004" The SOQ and any related information shall be delivered or mailed to: Los Angeles County Sheriff's Department Contracts Unit, Room 214 4700 Ramona Boulevard Monterey Park, California 91754-2169 Attention: Irma Santana, Contracts Unit It is the sole responsibility of the submitting Vendor to ensure that its SOQ is received on or before 3:00 p.m. (PST), on December 21, 2004, the submission deadline. Submitting Vendors shall bear all risks associated with delays in delivery by any person or entity, including the U.S. Mail. No facsimile (fax) or electronic mail (e-mail) copies will be accepted. This RFSQ will be open-ended. There will be an initial due date, stated above, and SOQ's not received by that date may not be reviewed initially, however, they will be reviewed at a later date to determine if they meet the qualifications listed in the RFSQ and are deemed to be responsive. MASTER AGREEMENT Appendix G of this RFSQ is a Sample Master Agreement that contains the terms and conditions of the services to be performed. The Master Agreement is a standard County Agreement and its terms are not negotiable.
